Travel Insurance Information

Travel insurance is recommended for domestic trips and required for international trips. Keep Your Heart Ministries does not provide this insurance. We recommend obtaining this insurance through Faith Ventures. Each individual must provide proof of obtaining travel insurance before embarking on an international trip with Keep Your Heart Ministries. Participants who do not purchase travel insurance for domestic trips assume all of the risks associated with travel.

I understand that Keep Your Heart Ministries does not provide travel insurance for my short-term domestic mission trip. I understand that by choosing not to purchase travel insurance I assume all of the risks associated with travel.

Standard of Conduct

- I agree to comply with all standards concerning the use of my cell phone while on the mission trip.
- I agree to abstain from engaging in any form of inappropriate conversation, foul language, etc.
- I agree to abstain from all forms of sexual immorality.
- I agree to abstain from the use or possession of any inappropriate substances.
- I agree to maintain a proper attitude of humility, service and cooperation at all times.
- I agree to conduct myself in a manner that will bring glory to God and edify those I work with and serve.
- I agree to respect the leadership of KYHM and respond appropriately to any and all requests.

Waiver Information

A signed waiver by this registrant will be required before the registration process is considered complete. A link to sign this waiver will be shown on the confirmation page.

Photo/Video Release

I grant Keep Your Heart Ministries the absolute right to copyright, re-use, publish, and republish by any medium, including electronically, any photos or videos of me (or my child) or in which I (they) may be included, that may be taken while participating in mission trip activities.

Deposit Information

A non-refundable deposit of $100 per person is required to reserve their place on the trip.

You will be given the option to pay this deposit using your debit/credit card or by check in the billing information section below.

If you plan to pay the deposit using a credit or debit card your card will be automatically charged.

If you plan to pay the deposit by check you will be given additional instructions when the form is completed.
